Photoshop Greyscale to RGB "scum dot"

I've run into a strange issue in Photoshop and am not quite sure  
where the problem lies.

Here's the situation:

Greyscale mode document in Photoshop, filled with white (K0%). Assign  
as source profile Black ink - US Web Coated Swop v2. Convert to  
profile, use Adobe RGB '98 as destination, Apple cmm, and relative  
colorimetric as intent, BPC on.

The resulting RGB values that I get are 255, 254, 253. Using Adobe  
ACE CMM, I get 255, 255, 255, and if I turn off BPC then I also get  
255, 255, 255 if using the Apple CMM.

So my question is, why would I not get RGB 255, 255, 255 from using  
the Apple CMM w/ BPC, and is this likely a glitch with the CMM?
